Before Your First Visit
Please bring the following to your first appointment:
- Completed new patient forms (download below or complete in office)
- Your dental insurance card(s) if applicable
- A photo ID
- List of current medications
- Any recent X-rays from your previous dentist (if available)

If you prefer paper forms, please arrive 15 minutes early to your first appointment and our front desk team will assist you.
